Fildena is a specific inhibitor of PDE5, an enzyme responsible for the breakdown of cGMP to 5’-GMP. Increased levels of cGMP stimulate vasodilation and facilitate the generation and maintenance of erections. These vasodilatory effects also help decrease symptoms of PAH. Sildenfail also exhibits some activity against PDE6 (10 times less potentcy compared to PDE5), a PDE isoform found predmoninantly in the retina. Fildena works by increasing the relaxation of the muscles of the penis. When an erection occurs, the smooth muscles of the penis and arteries surrounding the body should be relaxed. Fildena inhibits the cGMP-specific phosphodiesterase type 5 (PDE5) which is responsible for degradation of cGMP in the corpus cavernosum located around the penis. Penile erection during sexual stimulation is caused by increased penile blood flow resulting from the relaxation of penile arteries and corpus cavernosal smooth muscle. This response is mediated by the release of nitric oxide (NO) from nerve terminals and endothelial cells, which stimulates the synthesis of cGMP in smooth muscle cells. Fildena increases the effects of the blood pressure lowering medications. Patients taking nitrates should not receive Fildena.
Cimetidine (Tagamet), erythromycin, ketoconazole (Nizoral), itraconazole (Sporanox), atazanavir (Reyataz), and mibefradil (Posicor) can cause marked increases in the amount of Fildena in the body. Patients taking these medications should be observed carefully if sildenafil is used.
It is expected that Rifampin will decrease blood levels of Fildena and probably reduce its effectiveness.
Fildena has been tested and proven to be safe in men aged 85 years maximum. Fildena has not been tested in men younger than 18 years. It is not stated in the pediatric age group

Fildena is usually taken only when needed, 30 minutes to 1 hour before sexual activity. You may take it up to 4 hours before sexual activity. Do not take Fildena more than once per day.
